  1. This form is intended as documentation for reporting any parent, coach, player, or official whose actions are suspect of having violated the code of conduct.
  2. Incidents must be submitted on this standard form.
  3. These forms are available from your manger, division coordinator, and on the FGS website.
  4. The form should be completed with as much detailed information as possible.
  5. The completed form must be submitted within one week of the incident to an FGS Board member.
  6. Upon receipt, the Board will review the form and respond to you acknowledging the receipt of the report.
  7.  This form will be kept confidential and will only be reviewed by the FGS Board.
